2024-2025 registration page
To complete the TRM registration process:
- Completely fill out the online registration form below
- Turn in a proof of your sports physical exam clearance for 2024, Deadline for physicals to be received is October 20, 2024
- Turn in the completed 2024-2025 TRM parent waiver and requirements form and Behavior contract.
Forms may be scanned and emailed to the TRM program director Dave Busby at TRMBusby@gmail.com or returned to the school site leader. Site leaders are listed on the school tab of the website.
Parent waiver and requirements for student participation & Behavior Contract
Parents must sign these wavier forms and return them with their student to their school site leader.
All students are required to provide proof of a 2024 sports physical. Physicals may be acquired through a family doctor or through a school program. Golden Valley Health Clinic will provide a low cost physical for those students whose families qualify under the CHDP program.
Check the box in the form below to apply to receive a physical through GVHC. A representative from GVHC will contact applicant’s parents by phone to verify eligibility and to schedule an appointment.
